HOW DOES THE PLAN WORK?
The Medical Plan consists of three options,
namely gold, Silver and Bronze. When making your
selection you need to carefully consider the
annual limits as well as the benefit sub-limits
for each option and then choose the most
appropriate option for you and your family.
Membership of the Medical Plan is open to
individuals as well as employer groups and is
restricted to the age category of between o and
65 years. Children between the ages of o and 18
are classified as dependents of the principal
member unless they receive permission form their
parent/legal guardian to join the Medical Plan
as a principal member. Students between the ages
of 18 and 26 are required to provide proof of
their enrolment at a tertiary institution.
Principal member between the age of 60 and 65
are only permitted to renew their membership
subject to a medical examination. No new members
will be admitted after the age of 60.
LIFE ASSURANCE DEATH CLAIM PROCEDURE
As soon as the company has been informed of the
death of the insured, the net amount payable
upon proof of death and title are checked as
follows:
The proofs usually required are:
a. A certified death certificate
b. A declaration of identity
c. A formal letter
d. The original life Assurance document.
HOW DO I JOIN?
You will need to complete a membership
application form for you and your dependants and
return the completed form to your local
Provident office.
On acceptance you will be issued with a
membership card, a member guide and a master
policy.
Applications by individuals as opposed to
companies will be subject to strict
underwriting. Failure to disclose a pre-existing
condition will result in termination of
membership.
MODE OF PAYMENT OF PREMIUM
CONTRIBUTION CAN BE MADE BY ANY OF THE FOLLOWING
METHODS:
i. Direct payment by cash
ii. Salary deduction by company pay-roll
iii. Bank Direct Debit
iv. Money order or cheque |
